Case management has been variously classified as a skill group, a core function, service coordination, or a network of "friendly neighbors." Although it defies precise definition, case management generally can be described as a coordinated approach to the delivery of health, substance abuse, mental health, and social services, linking clients with appropriate services to address specific needs and achieve stated goals. The Consensus Panel that developed this TIP believes that case management lends itself to the treatment of substance abuse, particularly for clients with other disorders and conditions who require multiple services over extended periods of time and who face difficulty in gaining access to those services. This document details the factors that programs should consider as they decide to implement case management or modify their current case management activities. Unemployment and substance abuse may be intertwined long before an individual seeks treatment. Although the average educational level of individuals with substance abuse disorders is comparable to that of the general U. S. population, people who use substances are far more likely to be unemployed or underemployed than people who do not use substances. According to the U. S. Census Bureau, employment rates for the non- substance-using population ranged from 72.3 percent in 1980 to 76.8 percent in 1991. However, employment rates of the population with substance abuse problems before admission or at admission to treatment have remained at relatively stable, low levels since 1970, ranging from 15 to 30 percent. Most of the research on the employment rates of persons with substance abuse disorders has focused on opiate-dependent persons (usually heroin), and employment rates for other substance users may vary. Major research advances have substantially improved our understanding of the biology of HIV and the pathogenesis (i.e., origin and development) of AIDS. The pathogenesis of AIDS is now known to result from the ability of HIV to replicate at the rate of a billion new virions (viral particles) per day and nearly 10 trillion new virions over the course of HIV infection. This, countered by the ability of the body to produce CD4+ T cell lymphocytes (a primary target cell for HIV), sets the stage for the struggle between HIV and the immune system-a struggle that lasts from the first day of HIV infection to end-stage disease and death.

The Treatment Improvement Protocol (TIP) series fulfills the Substance Abuse and Mental Health Services Administration's (SAMHSA's) mission to improve prevention and treatment of substance use and mental disorders by providing best practices guidance to clinicians, program administrators, and payers. TIPs are the result of careful consideration of all relevant clinical and health services research findings, demonstration experience, and implementation requirements. A panel of non-Federal clinical researchers, clinicians, program administrators, and patient advocates debates and discusses their particular area of expertise until they reach a consensus on best practices. Lewy body dementia (LBD) is a complex and challenging brain disorder. It is complex because it affects many parts of the brain in ways that scientists are trying to understand more fully. It is challenging because its many possible symptoms make it hard to do everyday tasks that once came easily. Although less known than its "cousins" Alzheimer's disease and Parkinson's disease, LBD is not a rare disorder. More than 1 million Americans, most of them older adults, are affected by its disabling changes in the ability to think and move. As researchers seek better ways to treat LBD-and ultimately to find a cure-people with LBD and their families struggle to get an accurate diagnosis, find the best treatment, and manage at home.

Few people have heard of frontotemporal disorders, which lead to dementias that affect personality, behavior, language, and movement. These disorders are little known outside the circles of researchers, clinicians, patients, and caregivers who study and live with them. Although frontotemporal disorders remain puzzling in many ways, researchers are finding new clues that will help them solve this medical mystery and better understand other common dementias. The symptoms of frontotemporal disorders gradually rob people of basic abilities-thinking, talking, walking, and socializing-that most of us take for granted. They often strike people in the prime of life, when they are working and raising families. Families suffer, too, as they struggle to cope with the person's daily needs as well as changes in relationships and responsibilities.

Dementia is the loss of cognitive functioning-the ability to think, remember, or reason-to such an extent that it interferes with a person's daily life and activities. These functions include memory, language skills, visual perception, problem solving, self-management, and the ability to focus and pay attention. Some people with dementia cannot control their emotions, and their personalities may change. Dementia ranges in severity from the mildest stage, when it is just beginning to affect a person's functioning, to the most severe stage, when the person must depend completely on others for basic activities of daily living. Age is the primary risk factor for developing dementia. For that reason, the number of people living with dementia could double in the next 40 years as the number of Americans age 65 and older increases from 48 million today to more than 88 million in 2050. Regardless of the form of dementia, the personal, economic, and societal demands can be devastating.

At the end of life, each story is different. Death comes suddenly, or a person lingers, gradually fading. For some older people, the body weakens while the mind stays alert. Others remain physically strong, but cognitive losses take a huge toll. Although everyone dies, each loss is personally felt by those close to the one who has died. End-of-life care is the term used to describe the support and medical care given during the time surrounding death. Such care does not happen only in the moments before breathing ceases and the heart stops beating. Older people often live with one or more chronic illnesses and need a lot of care for days, weeks, and even months before death. The goal of End of Life: Helping with Comfort and Care is to provide guidance and help in understanding the unfamiliar territory of death. This information is based on research, such as that supported by the National Institute on Aging (NIA), along with other parts of the National Institutes of Health.

This TAP is intended for use by the disaster planning team of the behavioral health treatment program and others responsible for management and oversight of preparedness. This team may include program administrators responsible for developing and activating the organization's disaster plan, as well as senior staff members and clinicians who play leadership roles in developing and testing the plan, coordinating staff training of the plan, and activating the plan in an actual incident. The TAP also may be useful to executive directors and other members of why their involvement and support are so important. Objectives: The purpose of this study was to conduct a systematic review of the scientificmedical literature to identify, appraise and synthesize the evidence for the effects of omega-3 fatty acids in mental health. Evidence was sought to permit the investigation of three basic questions: the efficacy and safety of omega-3 fatty acids as (primary or supplemental) treatment of psychiatric disorders or conditions (e.g., symptoms alone); the association between intake of omega-3 fatty acids and the onset, continuation or recurrence of psychiatric disorders or conditions; and, the association between the fatty acid content of biomarkers and the onset, continuation or recurrence of psychiatric disorders or conditions. The latter two questions examined the protective value of omega-3 fatty acid content in the diet and/or blood lipid biomarkers. The impact of effect modifiers was examined as well. The results will be used largely to inform a research agenda.
