Depression is a major health problem that affects many older people, causing significant distress, disability, exacerbating existing medical conditions & resulting in earlier death & higher use of services. In depressed older people, depressed mood may be less commonly reported, while somatic symptoms (loss of appetite, lack of energy, irritability, sleeplessness, worrying, aches & pains) are more prominent. Elderly depressed people are more likely to have psychotic delusions than younger people. This profile has prompted the development of a separate instrument, the Geriatric Depression Scale, to measure depression in older people. Some aetiological pathways are also different in older people: in addition to psychosocial and genetic factors, late-life depression is also associated with cerebrovascular disease & its associated risk factors (high blood pressure, diabetes, smoking, increased serum lipid levels). The evidence for the effectiveness of a range of proposed treatments for depression in older people, including medical, psychological & alternative therapies & lifestyle changes. It is hoped that the book provides useful information that will help psychologists & psychiatrists.

Recent research, has documented a significant relationship between gender and psychological distress. Gender differences occur particularly in the rates of common mental disorders - depression, anxiety and somatic complaints. Research results was the finding that women have higher rates of mental health disorders, especially depression, than men. Gender is a critical determinant of mental health and mental illness. The morbidity associated with mental illness has received substantially more attention than the gender specific determinants and mechanisms that promote and protect mental health and foster resilience to stress and adversity. It is our intention to draw upon the existing research on psychotherapy and on the psychology of men to provide clear guidance about how to create positive, effective therapeutic outcomes with male clients. Some of the points suggested may cause clinicians to pause and question the manner in which they were trained and the way they practice psychotherapy with male clients. We consider such reflection a healthy way to evaluate how the mental health profession can effectively adapt to the needs of men. The book is organized into four sections.

Depression is a common disorder that involves depressed mood and/or near-complete loss of interest or pleasure in activities that were previously enjoyed;somatic (eg, weight change, sleep disturbance) & cognitive manifestations (eg, difficulty concentrating) are common. Depression may markedly impair the ability to function at work & to interact socially;risk of suicide is significant. Depressive disorders occur at any age but typically develop during the mid-teens, 20s, or 30s. Exact cause of depressive disorders is unknown, but genetic & environmental factors contribute. Sometimes depressive symptoms are caused by physical disorders (eg, thyroid or adrenal gland disorders, benign or malignant brain tumors, stroke, AIDS, Parkinson disease, multiple sclerosis) or use of certain drugs(eg, corticosteroids, some beta-blockers, interferon, reserpine, ...). Diagnosis is based on clinical criteria; physical disorders must be ruled out by clinical evaluation & selected testing (CBC; electrolyte, TSH, B12 & folate levels). Treatment involves psychotherapy & usually drugs; SSRIs are usually tried first, & if they are ineffective, other drugs that affect serotonin and/or norepinephrine may be tried.
